What is the Peruvian Health Affidavit and Geolocation Authorization Form?

The Peru Health form — required by the government for entry into Peru as part of the Covid-19 entry regulations — is an online form that asks for your personal information, travel information, and any symptoms you may be experiencing. All persons traveling into Peru (regardless of status: tourist, resident, Peruvian, adult or child, vaccinated or unvaccinated) were required to fill it out. It also served as a geolocation authorization, though the form only allowed a Peruvian number, so it was never clear how that system was actually being used.

When do I have to fill out the health form before traveling, and what do I do with it?

The Peruvian health affidavit must be completed before you travel — within 72 hours of flying — and you should keep a copy on hand as you travel. It is very important to complete this before arriving at the airport, as the form can be complicated and delay travel.

Additionally, you will need either a completed vaccine card (with the last dose given 14 days or more before), a negative PCR test, or a medical discharge if you have tested positive for Covid in the last 3 months.

What you will need on hand

The Peruvian health affidavit asks for lots of information. Be ready with:

  • Passport information
  • Date of arrival in Peru
  • Country of origin, written in Spanish
  • Destination department, province and district
  • Address where you will be staying (hotel, airbnb, etc.)
  • Flight information
  • Information of those you may be staying with

What you should know about the form

Sometimes the form doesn't load correctly in certain browsers. If you are using a VPN, turn it off and try reloading the page. You can also try a different browser — Chrome, Firefox and Safari all worked well for us.

Many people mention they can't find nationalities, states or cities in the drop-down lists. Everything is in Spanish. If names appear translated, switch off any translation program running in the background of your browser.

Where can I find the Peru Health Affidavit form?

The form is on the official Migrations website for Peru. Do not pay money to fill it out on another site — although tricky, the official form is free.

Steps to help you fill out the online health form

  1. On the first page, select your preferred language. Below the nationality field, if you're from the United States, select 'EE.UU' — the Peruvian abbreviation for USA. Select 'PAS' under 'documents' (short for 'passport'), enter your passport number in the 'Document Number' box, and input your date of birth. Note: birthdate is written DAY/MONTH/YEAR.   2. Confirm to move to the page for personal information, including destination within Peru.
  3. Fill in your name. Many countries use two last names — if you only have one, leave one blank and the system will accept it.
  4. Fill in your nationality. Country names appear only in Spanish (USA = EEUU, England = Inglaterra, and so on).
  5. Add your arrival date, tick international trip, add your flight number, then fill in country of origin (also in Spanish) and destination department, province and district. In the Cusco area use Cusco, Cusco, Cusco. If your real destination isn't listed, use Cusco — no one is checking this field.
  6. Phone number is the tricky part — it requires two Peruvian-format numbers. If you don't have one, any digits will be accepted. Format the first as "51" "xxx" "xxxxxx" and the second as "51" "xxx" "xxxxxxxx". Contact phone number
  7. Add your email in the required box.
  8. For 'real address', you can only use Peruvian departments, provinces and districts. Use your hotel address. When asked for 'real address after isolation' — use the same accommodation address again.
  9. The next step asks about people you'll be with at your address. This part can be left blank — we actually recommend leaving it blank as many travellers report the form failing to submit when it's filled in.
  10. Answer the symptoms question. We've never ticked a box as symptomatic, so we don't know exactly what happens if you do — but you'll potentially be asked not to travel.
  11. Tick the boxes for the declaration of oath and geolocation authorization. You'll be taken to the Health Affidavit page, which you can download to your phone or print. Carry it with you — you'll be asked for it more than once.
